June TARGET – Health Inequalities – South & Central

Aim: Work collaboratively on a plan on how to narrow and reduce Health Inequalities in Doncaster.

Objectives: 
Understand what Health Inequalities are
Consider what causes Health Inequalities and how they occur
What can primary care do to narrow Health Inequalities to effect change within Doncaster

Please note, this session includes time for practice teams to discuss an exercise together. In preparation for this you will you will need to prepare a room/meeting where you can meet as a whole team either in person or virtually, and write notes to record your discussion and feedback to the main meeting.
